New Patients

Welcome to Atlantic Foot & Ankle Associates! We are honored that you have entrusted your foot care needs to us. We believe that you are an important part of your own care team, and we want you to feel comfortable about all aspects of your relationship with us.

Good Information is Key

You can become more familiar with Atlantic Foot & Ankle Associates by visiting our staff page online. We have an expert group of podiatrists who believe in treating all your concerns in a caring manner. We maintain the highest levels of accreditation and stay updated on the latest treatment methods that bring the best results.

New patients can visit our offices page online to find details of our locations in Daytona Beach, Port Orange, Palm Coast, Orange City and Ormond Beach. The address, hours, and contact information for each office is available, as well as information on scheduling appointments, insurances accepted, and billing procedures. If you have more questions, please call the office phone number listed for that site, and our friendly staff will be happy to answer them.

You may also want to explore our online library of conditions before your first visit for information about your particular problem. Learning about the causes and treatment options can be informative and spark questions that you would like to have answered during your visit. We believe that education is key to making good decisions about your health.

Preparing for Your First Appointment

We need to collect certain information from new patients to provide the best care, so we request that you download and fill out the forms below before your first visit. You can do this at home where you have access to all your information. That will mean less stress when you come, and it also reduces the time needed off work on your appointment day.

During your first visit, we will need to obtain important background information including your medical history and current medications. We will also need to know the name of your Primary Care Physician (PCP) and the date of your last medical visit. Our staff will be happy to help you through the initial paperwork if needed. Once you are in our system, follow-up visits are very streamlined. We use an Electronic Health Record (EHR) system which allows us to easily update and maintain your health information. This system also allows you to securely access certain information online via the Patient Portal. This feature can be set up for you at your initial or follow-up visits.

Things you will need to bring to your first office visit include:

  1. Insurance Cards
  2. Photo I.D.
  3. Current medication list (please DO NOT bring in all your medications)
  4. Name of your Primary Care Physician and date last seen
  5. All of the patient forms you see listed below

*Be sure to write down all your questions before you come, as well, so we can answer them during your visit.
