Preparing for Your First Appointment
We need to collect certain information from new patients to provide the best care, so we request that you download and fill out the forms below before your first visit. You can do this at home where you have access to all your information. That will mean less stress when you come, and it also reduces the time needed off work on your appointment day.
During your first visit, we will need to obtain important background information including your medical history and current medications. We will also need to know the name of your Primary Care Physician (PCP) and the date of your last medical visit. Our staff will be happy to help you through the initial paperwork if needed. Once you are in our system, follow-up visits are very streamlined. We use an Electronic Health Record (EHR) system which allows us to easily update and maintain your health information. This system also allows you to securely access certain information online via the Patient Portal. This feature can be set up for you at your initial or follow-up visits.
Things you will need to bring to your first office visit include:
- Insurance Cards
- Photo I.D.
- Current medication list (please DO NOT bring in all your medications)
- Name of your Primary Care Physician and date last seen

Atlantic Foot & Ankle Associates specializes in many podiatric concerns, including bunions, ingrown toenails, heel pain, fungal toenails, neuromas, sports injuries, and diabetic foot care. We provide excellent foot care for everyone from infants to the elderly. Our goal is to offer conservative treatments such as orthotics, platelet rich plasma therapy, and shockwave therapy whenever possible, and reserve surgery for the more difficult cases.
